Eligibility Paper Work Check List

We have four important forms we need from athletes before starting practice next Monday, March 30th. Click on the first two for printable versions.

1.Physical
2.District 199 Eligibility Form
3.$70 Fee Checks can be made out to ISD 199. Put Track and athletes name in the memo portion. (Upon request of parent this fee can be waived. We do not any student excluded based on finances.)
4.Team rules contract. See earlier post. This can be cut and pasted into a Word document for printing.
